The Carpenters Center for Health will be conveniently located on the 3rd floor of the Chicago Regional Council of Carpenters’ Lisle campus – 4979 Indiana Avenue, Lisle, IL.
+ What will the hours of operation be for the Carpenters Center for Health?
The Carpenters Center for Health’s will be opening in June 2018, and the hours of operation will be Monday, Tuesday, Wednesday and Friday 8 a.m. – 5 p.m., and Thursday 10 a.m. – 7 p.m.
+ What type of medical staff will work at the Carpenters Center for Health?
The Carpenters Center for Health staff will include a Doctor as the Medical Director of the Health Center, Nurse Practitioner, Registered Nurse, Health Case Care Manager (also a Registered Nurse), Medical Assistants, health center manager and skilled support staff. Premise Health will carefully screen and staff the Carpenters Center for Health with qualified providers ready to deliver exceptional care to you and your family. Anyone working at the Carpenters Center for Health will be an employee of Premise Health and not an employee of CRCC.
+ What is a patient centered medical home?
A patient centered medical home (PCMH) is not a building, a “home”, or a hospital, but rather an approach to providing you and your family comprehensive healthcare. It’s a powerful new effort focused on treating you, the “patient”, and giving you the best care at all times. The PCMH model is all about strengthening the provider-patient relationship; providing whole-person, coordinated care, enhancing the healthcare experience and outcomes through better communication, expanding hours and access, and encouraging active patient participation in healthcare delivery. At the Carpenters Center for Health, you will be surrounded by a dedicated team of health professionals working together to meet all of your individual healthcare needs. The Health Center's goal is to make it easy and comfortable to get the care you need in a way that works best for you, while delivering the highest quality of healthcare.
+ Will I have to use the Carpenters Center for Health?
Receiving care at the Carpenters Center for Health is completely voluntary. The choice is yours. You can continue to receive care from your regular doctor if you prefer but the Carpenters Center for Health will be a convenient, no cost option. If you choose to receive primary care services at the Carpenters Center for Health but need a referral to a specialist, the staff will help connect you to an appropriate in-network specialist for your health condition.

Premise Health is the organization that will operate and staff the Carpenters Center for Health. They are one of the nation’s leading managers of onsite health clinics. + What types of healthcare services will the Carpenters Center for Health offer?
The Carpenters Center for Health is a patient centered medical home that will provide comprehensive services focused on strengthening the provider-patient relationship, providing whole-person, coordinated care. Services will include the following:
- Primary Care Services
- Annual physicals
- Lipid profiles, blood glucose and blood pressure screenings
- Flu shots, immunizations and vaccinations
- School and sports physicals
- Skin, breast and prostate cancer screenings
- Allergy management
- Wellness exams
- Acute/Urgent Care
- Common cold
- Sinus infection and stuffy nose
- Seasonal allergies
- Minor surgical procedures
- Disease/Condition Management
- Diabetes, high cholesterol, hypertension
- Asthma, COPD
- Back pain, migraine, osteoarthritis

- Limited generic medications will be provided on site as part of your
visit with the provider in support of condition management

+ How will my current medical provider know I am receiving care at the Carpenters Center for Health?
After you have been treated at the Carpenters Center for Health, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
+ Will the Carpenters Center for Health have a pharmacy?
The Carpenters Center for Health will not have a full‑service pharmacy; however, it will carry a variety of medications on site for treatment of acute illness such as a sinus infection, strep throat, etc. Limited starter generic medications will be provided on site as part of your visit to the Health Center in support of condition management (for example, a cholesterol lowering medication for the diagnosis of high cholesterol). The staff will be able to prescribe medications and send the prescriptions to your regular pharmacy for pickup at your convenience.
+ How much does it cost to use The Carpenters Center for Health?
All services provided at the Health Center are free of charge to eligible CRCC members and their dependents.
+ Who is eligible to use the Carpenters Center for Health?
Access to the Carpenters Center for Health will be available to members (including spouses and dependent children, ages 2 and up) who are eligible and enrolled in comprehensive medical benefits under the Chicago Regional Council of Carpenters Welfare Fund’s Active, Retiree and Low Cost Plans.
+ Can I use the Carpenters Center for Health if I’m not currently eligible for benefits?
No. To use the Carpenters Center for Health you and your dependents must be eligible for benefits and enrolled in comprehensive medical benefits under CRCC’s Welfare Fund’s Active, Low Cost or Retiree Plans.
+ Can I continue to use the Carpenters Center for Health when I retire?
Yes, as long as you are eligible for and enrolled in the Comprehensive Medical benefits under the Retiree Plan.
+ Can I use the Health Center if I’m not eligible for benefits but I’m willing to pay for services?
If you are not eligible for benefits, you may not use the health center.
+ How will I make an appointment at the Carpenters Center for Health?
When the Health Center opens, you can call or visit the center during regular hours of operation to make an appointment. There will also be an online patient portal where you can make your appointments via your computer, phone or tablet. Stay tuned for details on how to register.
+ Will the Carpenters Center for Health accept walk-ins?
Walk-in patients are welcome, but it’s always preferable to schedule an appointment. In most cases, same-day appointments are available.

It is referred to as My Premise Health.
+ What is My Premise Health?
My Premise Health will be your new patient portal which is part of the Epic’s electronic health records platform. My Premise Health is a secure online and mobile tool that will put your health information in your hands. My Premise Health will connect you with your healthcare experience by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, test results, and more. + Will my health information be secure? Who can access to my personal health information?
Your personal health information is confidential and your data will be kept secure. The Carpenters Center for Health will operate in accordance with HIPAA and work diligently to protect all health records. Premise Health cannot share any personal health information without your permission. Rest assured, your personal health information will not be shared with your employer. Additionally, Premise Health will operate through a private network and software systems that cannot be accessed by other members or CRCC’s staff. Notice of privacy practices will be available for review within the Carpenters Center for Health.
