The Board of Trustees of the Chicago Regional Council of Carpenters Welfare Fund (CRCC) cares about the health and well‑being of their members and wants to ensure that they have exclusive access to quality healthcare without any potential cost barriers. We believe being healthy is about being at your best, every day – and knowing where to turn when you don’t feel quite right. That's why CRCC has partnered with Premise Health to create the Carpenters Center for Health, your family's health partner.

The Carpenters Center for Health is an innovative patient centered medical home available to eligible members and their dependents, located on the Chicago Regional Council of Carpenters’ Lisle campus. The Health Center provides a wide array of high quality primary care and preventive services along with acute care. In addition, there are condition management and patient education services available. All services at the Health Center are at no cost to members and their eligible dependents.

Who is eligible to use the Carpenters Center for Health? Access to the Carpenters Center for Health is available to members (including spouses and dependent children, ages 2 and up) who are eligible and enrolled in comprehensive medical benefits under the Chicago Regional Council of Carpenters Welfare Fund’s Active, Retiree and Low Cost Plans.

 
 

What does a patient centered medical home mean for you and your family?

A patient centered medical home (PCMH) is not a building, a “home”, or a hospital, but rather an approach to providing you and your family comprehensive healthcare. It’s a powerful new effort focused on treating you, the “patient”, and giving you the best care at all times. The PCMH model is all about strengthening the provider-patient relationship; providing whole-person, coordinated care, enhancing the healthcare experience and outcomes through better communication, expanding hours and access, and encouraging active patient participation in healthcare delivery.

At the Carpenters Center for Health, you are be surrounded by a dedicated team of health professionals working together to meet all of your individual healthcare needs. The Health Center's goal is to make it easy and comfortable to get the care you need in a way that works best for you, while delivering the highest quality of healthcare.

From Hwy 34 / Ogden Avenue, take Indiana Avenue heading south (for an added marker, Molex campus begins at the corner). At the cul-de-sac, turn left, and the building is on the right. The sign says, “Carpenters Regional Center.” Drive around to front of building to access the Health Center.
