The Carpenters Center for Health staff includes a Doctor as the Medical Director of the Health Center, Nurse Practitioner, Registered Nurse, Health Case Care Manager (also a Registered Nurse), Medical Assistants, health center manager and skilled support staff. Premise Health has carefully screened and staffed the Carpenters Center for Health with qualified providers ready to deliver exceptional care to you and your family. Anyone working at the Carpenters Center for Health is an employee of Premise Health and not an employee of CRCC.

+ Do I have to use the Carpenters Center for Health?

Receiving care at the Carpenters Center for Health is completely voluntary. The choice is yours. You can continue to receive care from your regular doctor if you prefer but the Carpenters Center for Health is a convenient, no cost option. If you choose to receive primary care services at the Carpenters Center for Health but need a referral to a specialist, the staff will help connect you to an appropriate in-network specialist for your health condition.

+ How will my current medical provider know I am receiving care at the Carpenters Center for Health?

After you have been treated at the Carpenters Center for Health, you may request any records be sent to another medical provider. If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

+ Does the Carpenters Center for Health have a pharmacy?

The Carpenters Center for Health does not have a full‑service pharmacy; however, it does carry a variety of medications on site for treatment of acute illness such as a sinus infection, strep throat, etc. Limited starter generic medications are provided on site as part of your visit to the Health Center in support of condition management (for example, a cholesterol lowering medication for the diagnosis of high cholesterol). The staff is able to prescribe medications and send the prescriptions to your regular pharmacy for pickup at your convenience.

+ How much does it cost to use The Carpenters Center for Health?

All services provided at the Health Center are free of charge to eligible CRCC members and their dependents.

+ Who is eligible to use the Carpenters Center for Health?

Access to the Carpenters Center for Health is available to members (including spouses and dependent children, ages 2 and up) who are eligible and enrolled in comprehensive medical benefits under the Chicago Regional Council of Carpenters Welfare Fund’s Active, Retiree and Low Cost Plans.

+ Can I use the Carpenters Center for Health if I’m not currently eligible for benefits?

No. To use the Carpenters Center for Health you and your dependents must be eligible for benefits and enrolled in comprehensive medical benefits under CRCC’s Welfare Fund’s Active, Low Cost or Retiree Plans.

+ Can I continue to use the Carpenters Center for Health when I retire?

Yes, as long as you are eligible for and enrolled in the Comprehensive Medical benefits under the Retiree Plan.

+ Can I use the Health Center if I’m not eligible for benefits but I’m willing to pay for services?

If you are not eligible for benefits, you may not use the health center.

+ How do I make an appointment at the Carpenters Center for Health?

You call or visit the center during regular hours of operation to make an appointment, or simply schedule online.

Same-day appointments may be available, but we suggest you call first.

It is referred to as My Premise Health.

My Premise Health is your new patient portal which is part of the Epic’s electronic health records platform. My Premise Health is a secure online and mobile tool that puts your health information in your hands. My Premise Health connects you with your healthcare experience by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, test results, and more.

+ Is my health information secure? Who can access to my personal health information?

Your personal health information is confidential and your data will be kept secure. The Carpenters Center for Health operates in accordance with HIPAA and works diligently to protect all health records. Premise Health cannot share any personal health information without your permission. Rest assured, your personal health information will not be shared with your employer. Additionally, Premise Health operates through a private network and software systems that cannot be accessed by other members or CRCC’s staff. Notice of privacy practices are available for review within the Carpenters Center for Health.
